Step 1: Request an Appointment
Patients or caregivers should call 1-215-563-4733, fill out an online contact form or send an email to firstname.lastname@example.org to begin the process of your medical history review and appointment scheduling.
Our business hours are Monday-Friday, 8:30 a.m. to 5:00 p.m. Eastern Standard Time.
Once your request is received, you will be contacted by a PIM patient services coordinator to help you through the process of submitting your medical records. Please have the following information available:
Date of birth
Country of residence
Payer information and copy of insurance card, if applicable
All relevant medical records
Diagnosis and/or symptoms
Prepare your Visa
Make your Travel
Step 4: Appointment Confirmation
Once a deposit has been received or payer information has been confirmed, your patient services coordinator will send you a letter confirming your appointment with the name of your doctor, the date, time and location of your appointment and any other important information you need before traveling.
Telemedicine Patients will receive confirmation along with instructions on how to access the portal.
